USA BMX / BMX Canada is the policyholder for Accident Insurance for all registered members of USA BMX / BMX Canada enrolled in the voluntary annual buy-up option, participating in activities sponsored or supervised by USA BMX, including injuries sustained during events or while practicing on sanctioned USA BMX / BMX Canada racetracks. Coverage is subject to the terms, conditions, and exclusions of the policy. Coverage is underwritten and claims are serviced by Mutual of Omaha Insurance Company and facilitated by Spot.
The first and last name should represent the individual registered as a USA BMX / BMX Canada member. If you are signing a minor up for the policy please include their name inside the form.
